Information We May Collect

We collect and use information we believe is necessary to provide services to clients. Clients are asked to sign various release forms that allow us to gather information from other agencies and schools that may have provided services in the past. We treat this information as private and confidential in the same manner that we treat the information gathered directly from clients.

How We Use Information

We use the information we gather about clients to help develop treatment plans and educational plans, and to communicate with those involved in payment/contracting for the services provided to clients.

Information Disclosure

We share information about you and your child with Silver Springs treatment staff members. Other Silver Springs staff members may have access to information about you and your child as a part of their job function--such as secretarial/clerical work, the development and management of database information, and the preparation of billing information. We only share information on a need-to-know basis. We maintain physical, electronic and procedural safeguards to protect your information. Information about you may be disclosed to regulatory or other enforcement organizations as required by law.

Information gathered during the course of treatment may be shared with your insurance company or the managed-care organization responsible for funding services. All release authorization forms are time limited and may be revoked by you at any time.

You (or someone who legally represents you) have the right to request a copy of any information we have about you or your child, which we will provide to you unless prohibited by court order. You may ask us to communicate with you in any manner or at any place of your choosing (for example, by mailing materials to a P.O. box instead of to your home address). Any uses or disclosures of information other than those mentioned above will be made only with written authorization.

Accuracy of Information

If you receive information that you feel is incorrect, please speak first with your social worker followed by the Social Service Director so that we may make the appropriate changes.
